New Patients
New patients may register if they have moved into our practice catchment area, as displayed on the map. This area covers a 6-mile radius from the surgery, extending to an 8-mile radius to the west.
Please contact us to confirm whether your home is within our boundary.

Step 1
Gather Your Documentation
Please bring your completed form with:
• Proof of address: A utility bill from the last 3 months
• Photo ID: Your passport or driving licence
• Residency status: A visa or Home Office letter (non-UK residents)
The Archway Surgery welcomes all patients, regardless of their background or personal circumstances. If you use regular medication and are registered in Northern Ireland, please request a 28-day supply from your current GP before moving.
Joining the Practice
Step 2
Fill in the Registration Form
Patients transferring from a practice in the UK must complete form HS200. New residents not yet registered in either region should use form HSC-R1. Forms are available at our reception.
Step 3
Submit and Verify
Please visit reception between 8:30am–12:30pm or 1:00pm–6:00pm (Mon–Fri). Our team will review your documents to confirm you live in the catchment area. No appointment is required, but an introductory meeting with a GP or nurse is available if you have specific clinical needs.
