‘Sick lines’ for employees

Use form SC2, known as a ‘self-certificate’, if you are an employee, you’ve been sick for 4 or more days in a row and need to claim Statutory Sick Pay (SSP) from your employer.  To access an SC2 form click here.

When you’ve filled in the form give it to your employer. It will help them decide if you can get SSP. Keep a copy for your own information.

The SC2 form lasts for 7 days.  If you have been unfit to work for more than 7 days and have already submitted an SC2 form to your employer, you will require a med3 ‘fit note’, which is issued by a GP – please contact the surgery.
