How to register

The Practice welcomes new patients that have moved into the Armagh area. Click here to see our practice boundary.

All new patients will be asked to complete a set of forms and a health questionnaire before they are allocated an appointment slot with a Practice Nurse to register.

Please ensure that forms are completed fully and all necessary documents are included before returning them to reception staff.

Please note that patients are now registered with the Practice rather than with a particular GP, although we would encourage you to consult one doctor for chronic problems as this helps to ensure continuity of care.

The Northern Ireland New Entrants Service (NINES)

NINES offers nurse-led health care advice and initial health assessments for adults and children registering with a GP for the first time.

NINES is a first point of contact to the health service for new migrants, asylum seekers and refugees, offering health promotion advice and information on how to register with a GP and access other services they may need.

Application for a medical card

In order to obtain a medical card you need to fill in an application form, which is called HSCR1 registration form. Please click here to complete the online form on the Health and Social Care Business Services website
